This is due to the nozzle not being tight enough against the heat break inside the hotend. Once it's at temp, you might notice that you can tighten it a bit more.

You'll need to convert it to an .stl file first, most CAD Programs have an export option for that.
Then you can import the .stl file into your slicer (Slic3r, Cura, Simplify3D, etc.) to create the gcode file for your printer.

It looks like your nozzle is a bit too far from the bed, I'd recommend re-leveling.

Your nozzle is too close to the bed. Try re-leveling with a piece of printer paper. There should be a little friction between the paper and nozzle when it's between the nozzle and bed.

It sounds like you have either a partial clog in the nozzle, or the heatsink fan on the hotend isn't working and you're having heat creep, causing a clog. Since it doesn't happen until a few layers in, I'd guess the second one.
